Our mission at So Cal HHA home health services is to provide the utmost care and attention to each and every patient. Our goal is to improve the patients’ quality of life through the care and professionalism of our dedicated team of nurses in conjunction with the plan of care their doctor has set in place.
Wound Care Program is a specialty program centered not only on the treatment and healing of wounds but also on the patient's overall health. Our nurse wound specialists are certified and provide individualized and expert care under a patient's physician order. Continuous assessment and reassessment of wounds, patient care and change in the treatment plan as appropriate are implemented to ensure patients overall well-being and progress.
PHILOSOPHY
At So Cal HHA we operate with the philosophy, every patient should receive care in the safest, most appropriate and least restrictive setting possible. We want our patients to live comfortably, safely, and healthy in the ease of their home.
SPECIFICATIONS
Our dedicated on-field staff are composed of RNs, LVNs, and CHHAs who each have specific specializations in patient care. They are able to provide; skilled nursing assessments, wound care, injections, catheter care, ostomy care, intravenous fluid/medication management, diabetes mellitus care, vital signs monitoring, dressing change as well as bathing, among other necessities. They are also trained to educate patients and their families about safety, medications, diet, nutrition, and disease.
