Who Can Refer?
Anyone can request Hospice services with a physician's order and verification that there is an illness with a life-limiting prognosis. Care is available for patients with a prognosis of six months or less, such as patients with Terminal Cancer, end-stage Heart Disease, ALS, Chronic Obstructive Pulmonary Disease, Renal Failure, AIDS and Dementia. Patients do not need to be homebound in order to receive Hospice Care.
Pathways For Grief & Loss
Our path way for grief and loss is a tool to help families cope with the loss of the their loved. While we each choose our own path in life, and in grief, sometimes we need someone to guide us and walk by our side. Our program is here to assist any individual in our community -regardless of hospice involvement - who is grieving the death of a loved one or dealing with a serious illness. This includes support for family members and caregivers. Our grief counselors help individuals gain strength and warmth through connection, understanding, and support.

The Cost
The Medicare Hospice benefit covers all services, medications, supplies and equipment related to the terminal illness. Other insurances may offer some coverage for Hospice, but may not cover the entire cost. If a patient has no insurance or partial coverage, we will work with each patient on an individual basis. When it comes to managing the health needs of a loved one, there really is no place like home.
